Transition Worker (11997)
Transition Worker.
Salary: £25,000 per annum (pro-rata).
Location: Northeast England, Homebased (with regular travel to visit children and young people in the Northeast area).
Area of work will be Northeast England including Hull, Newcastle, Sunderland, Middlesborough and Durham.
Contract/Hours: Part Time Fixed Term Contract- 8 months – 22.5 hours a week – over 3 days Monday to Friday (Though some flexibility may be required).

A bit about the role
Fostering Northeast is a small and developing service that offers fostering placements using a therapeutic framework. As a Transition Worker, you will support care experienced people aged 16-24 with significant transitions. This might involve preparing them to move into independent living or supported accommodation. For young people moving on, this may be a move onto their own tenancy/suitable accommodation, or another planned destination.
Where young people are moving towards more independence you will assist them to develop the necessary life skills to manage their own tenancy independently, and responsibly and to budget effectively.
As a Transition Worker you will also support young people with skills to Transition to new environments whilst they live with their carers – this will include supporting them to move to college or exploring work-based options and experiences.
It will be your role to build a trusting relationship with the young people and support them to sustain positive relationships with other adults involved with them, as they move into the next stage of their lives. Where young people are moving on to live independently you will assist them to develop the necessary life skills to manage their own tenancy independently, and responsibly and to budget effectively.
How you'll help to create brighter futures
As a Transition Worker you will offer support and advice to our young people to overcome barriers that can include housing, homelessness, confidence and self-esteem issues, poor social skills, lack of qualifications and employment opportunities. You will also support young people with emotional wellbeing and provide resilient and nurturing support, even in challenging and demanding situations.
Key responsibilities of your role will be to; -
- Engage and build trust with young people to conduct assessment of their needs.
- Managing a case load of Young People across Northeast England.
- Link up with agencies so our young people feel safe and secure in their own home.
- Support young people to overcome their barriers to training, education or employment and support in preparation for a placement/job offer, open bank account, planning travel routes etc.
- Collaborate with employers, colleges, volunteering opportunities and training providers.
- Attending meetings with other key individuals involved in the care of a service user and completing relevant paperwork and reports.
- Be able to work within and support other areas of a young person's plan.
- Be able to reflect on Safeguarding concerns and engage in relevant discussions to address vulnerabilities.

We're looking for someone with great planning and organisational skills. Do you have the ability to communicate effectively and find it easy to build and engage with young people and professionals? If so, we would love to hear from you!
Some other requirements we have for the role are: -
- Level 3 NVQ, or working towards, or relevant experience/life experience.
- A knowledge of the issues and barriers facing looked after young people.
- The ability to use a variety of tools and approaches to support young people to express their feelings and views.
- Experience of working as part of a multidisciplinary team.
- Confidence in IT systems.
- Experience in developing support plans.
- Up to date knowledge of safeguarding policies and procedures.
- Access to a suitable method of transport to travel to various settings to conduct your role (essential).
